Ohio registration, step by step:

Get an Ohio Medical Marijuana Card.
It starts with a CTR physician, and Ohio charges no state fee.

To get an Ohio medical marijuana card, you need a qualifying condition and a recommendation from an Ohio physician who holds a Certificate to Recommend, and Ohio charges no state fee.

Ohio’s program dates to House Bill 523, effective September 8, 2016, and the Division of Cannabis Control now also oversees adult-use sales, which began on August 6, 2024.

One thing sets Ohio apart when you get a medical marijuana card in Ohio: your physician, not you, starts the state registration.

That happens after an exam with a CTR physician, who may not recommend cannabis for their own use or for a family member. To be eligible for registration you need to be an Ohio resident, 18 or older, with a qualifying medical condition listed by the state. Patients under 18 need a parent or legal guardian to consent and register as their caregiver.

Schedule a visit with an Ohio Marijuana Doctor who holds a CTR for your initial evaluation.

Once an Ohio CTR physician confirms your qualifying condition, the physician enters you in the state registry. An active Ohio registration and a current recommendation let you buy, possess and use medical cannabis within Ohio’s limits. The exam can be in person or by telehealth, and this page explains each step after it.

To buy medical marijuana in Ohio, you must first be registered in the Patient and Caregiver Registry. You’ll need to see a practitioner who is qualified to recommend cannabis to a patient for this. Only a physician with a CTR, or the physician’s delegate, can submit that registration. After the physician confirms a qualifying condition, you receive a recommendation for up to a 90-day supply.

Twenty-one come from Ohio Revised Code 3796.01, and the State Medical Board added the other five: cachexia, Huntington’s disease, irritable bowel syndrome, spasticity and terminal illness.

Who can register as a patient in Ohio?

Ohio residents aged 18 or older with one of the state’s qualifying conditions and a physician’s recommendation can register. A patient under 18 is registered by a parent or legal representative, who becomes the caregiver; caregivers must be 21, or 18 if they are the patient’s parent.

The State Medical Board of Ohio has also found arthritis, chronic migraines, complex regional pain syndrome, degenerative disc disease and lupus where pain is present to be covered by chronic or intractable pain, but your physician decides whether your own case meets that condition.

What ID does my Ohio physician need to register me?

Your Ohio driver’s license, Ohio ID card or another ID number the DCC accepts. The physician or a delegate uses it to submit your registration (ORC 3796.08(A)(1)).

Does a qualifying diagnosis alone get me an Ohio card?

No. An Ohio physician must first examine you, in person or by telehealth under ORC 4743.09, and decide to recommend medical marijuana. After that, the DCC expects at least one in-person or telemedicine visit a year.

Where do I find an Ohio Medical Marijuana Doctor?

Can I grow my own medical marijuana if I have obtained my ID card?

Yes, if you are 21 or older: Ohio allows six plants per adult and twelve per home, grown in a secured, enclosed area out of public view. Medical patients under 21 may not grow.

Does insurance cover medical cannabis?

Not required. Ohio’s cannabis law, Revised Code chapter 3796, does not require health insurers to pay for medical marijuana, so ask your insurer before you count on coverage.

How to Obtain A Medical Marijuana Card in Ohio?

State residents who have a recommendation from an Ohio doctor with a Certificate to Recommend can qualify for a medical cannabis card.

If you are a state resident who is 18 and older you will need to be certified by a qualified medical marijuana doctor.
Qualifying patients under 18 can apply for a medical marijuana card in Ohio but are required to have an adult caregiver.

Make an appointment to be evaluated for a medical marijuana card with an Ohio physician who holds a CTR. Please bring any relevant medical records with you to the appointment. During the evaluation, the physician decides whether you have a qualifying condition. If the physician confirms one and recommends medical marijuana, the physician or the physician’s delegate submits your application to Ohio’s medical marijuana Patient and Caregiver Registry.

How long does an Ohio registration last?

One year: it expires on the last day of the month it was issued, a year later. The DCC sends a notice 45 days ahead, you may renew up to 30 days before expiry at no state cost, and any cannabis left must be disposed of within 7 days after expiry.

Do I still need a registry card now that adult-use sales are legal?

Yes, to buy as a patient. A medical sale needs an active registry card, an active recommendation and a government photo ID. Adults 21 and over may buy adult-use cannabis without a card but pay the 10% excise tax.

Does Ohio charge a fee for the medical marijuana card?

No. Registering with the DCC costs nothing to activate or renew, for patients and caregivers alike.

Can I buy at a dispensary before my Ohio registration is active?

Not as a patient. Medical purchases start once the registry card is activated; adults 21 and over may buy adult-use cannabis in the meantime. Your registration must reach the DCC complete within 90 days of the recommendation date.

I lost my Ohio card. How do I get it back?

If your card has been lost you’ll need to contact the MMCP by phone at 1-833-464-6627, or by email at [email protected] and provide your first name, last name, date of birth, and preferred email address for your Registry profile. The registry card is digital, so you can also sign in at ohiomedicalmarijuanaregistry.com to view it.

Will anyone know I am an Ohio patient?

Your participation in a medical marijuana program is confidential and protected by Ohio law: the DCC may not make public any information that identifies a patient (ORC 3796.08).

What kind of life change should I report to the DCC?

Report any change to your registration details, such as name, address, email or phone, to the DCC within 30 days.

What does an Ohio registration allow that an adult-use purchase does not?

Registration lets patients aged 18 to 20 buy medical cannabis, which adult-use buyers must be 21 to do, and the DCC lets a patient buy up to four days of the daily limit in one transaction.

Did Ohio ever charge a patient card fee?

Yes. Until March 3, 2024, Ohio charged patients $50 a year and caregivers $25 to register. From March 4, 2024 the fee was one cent, and the DCC now states there is no activation or renewal cost for state registration.

What reasons are there for application denial?

Your application can be denied if

Your medical condition was not verified
You do not meet the age requirements and do not have a caregiver
Your registration was incomplete and not fixed within 90 days

Can a patient under 18 register in Ohio?

Yes. A parent or legal representative gives consent to the physician and registers as the caregiver (ORC 4731.30(C)(2); OAC 1301:18-10-01 and 1301:18-10-02). The patient may not make purchases, and vaporization is not authorized for a patient under 18.

How do I know my Ohio registration went through?

You get an email from the Patient and Caregiver Registry once your physician submits the registration. Follow it to review your details, accept the legal statement and activate the card; until you do, you cannot buy as a patient.

What types of medical cannabis products are approved for use in Ohio?

The state of Ohio allows the use of the following forms of medical marijuana

Ohio law bans smoking or burning medical marijuana. Vaping is allowed only on private residential or agricultural property and never for patients under 18.

Can my Ohio physician renew the recommendation without a new exam?

Up to three times. Each renewal adds up to 90 days, so a recommendation can cover 360 days before a new exam; the DCC describes this as a 90-day supply with three refills.

How many dispensaries are licensed in Ohio?

As of October 8, 2026, the Division of Cannabis Control lists 231 licensed dispensaries on its website.

Which stores can sell to Ohio patients?

Only at DCC-licensed dispensaries. On October 8, 2026, every one on the DCC list was licensed for both medical and non-medical sales. Bring your active registry card and a government photo ID.

Can adults without a card buy cannabis in Ohio?

Yes, adult-use only. Any adult 21 or older may buy adult-use cannabis, sold since August 6, 2024, while medical sales remain for registered Ohio patients.

What is Ohio’s daily purchase limit?

Each day you may buy 2.5 ounces of plant material and 15,000 mg of THC in other products, or four days’ worth in one visit, and keep no more than a 90-day supply at home.
