Getting a medical marijuana card in St. Louis begins with a certification from a Missouri-licensed physician or nurse practitioner, dated within 30 days of your application. Getting a Medical Marijuana Card in St. Louis
Missouri law covers cancer, epilepsy, glaucoma, intractable migraines, HIV/AIDS and terminal illness. Chronic conditions that cause severe, persistent pain or muscle spasms also qualify, and so does a debilitating psychiatric disorder such as PTSD when a Missouri-licensed psychiatrist diagnoses it.
A physician or nurse practitioner may also certify other chronic or debilitating conditions, including Crohn’s disease, sickle cell anemia, hepatitis C and agitation of Alzheimer’s disease. Missouri Medical Marijuana Card Program
Medical marijuana became legal in Missouri when 65.6% of voters approved Amendment 2 in November 2018, and the program now sits in Article XIV of the state constitution. To join the program, you need a certification from a Missouri-licensed physician or nurse practitioner, dated within 30 days of your application. You then apply online with the Missouri Department of Health and Senior Services. Adults 21 and older may also buy adult-use marijuana, limited to 3 ounces per purchase, while a medical card carries the 4% tax rate and the 6 ounce limit every 30 days.

There are two costs: the physician evaluation and the state ID card fee of $28.92, paid directly to the Missouri Department of Health and Senior Services. Your state card is valid for three years.
Missouri Medical Marijuana Laws
Cardholders can buy up to 6 ounces every 30 days and possess a 60-day supply. The state taxes medical sales at 4% and adult-use sales at 6%, and local governments can add an adult-use tax of up to 3%. Regular sales tax applies to both. Adult-use shoppers are limited to 3 ounces per purchase. Growing at home requires a separate cultivation card.
St. Louis Marijuana Dispensaries
DHSS publishes a list and map of licensed dispensaries, and a licensed shop shows a Verified Dispensary decal or its state license, so St. Louis patients can check a shop before they go. City of St. Louis Ordinance 71608 put an extra 3% city tax on adult-use (non-medical) marijuana sales to voters in April 2023, and it does not cover medical sales.

How do I get a medical marijuana card in St. Louis?
A Missouri-licensed physician or nurse practitioner certifies you first, and your online application to the Missouri Department of Health and Senior Services (DHSS) must follow within 30 days of that certification date. You will select the certification your practitioner filed, add a photo ID and a color photo from the last 3 months, and pay the fee. DHSS has 30 days to approve or deny a complete application.
Can I get my Missouri medical card evaluation online?
Yes. Under RSMo 191.1146, as amended by HB 2372 in 2026, a physician may use telemedicine when the standard of care does not require an in-person visit. Before certifying you by video, the physician must review your history and do an exam sufficient for a diagnosis.

A Missouri patient ID card stays valid for three years. To renew, apply no sooner than 60 days and at least 30 days before it expires, and attach a new certification from a Missouri-licensed physician or nurse practitioner.
Do I need to be a Missouri resident?
No. Missouri no longer requires proof of residency for a patient application. You still need a valid government-issued photo ID. Your card is only valid under Missouri law, so it does not protect you in states where marijuana is illegal, including Kansas.
How much marijuana can I buy with a medical card in Missouri?
The patient limit is 6 ounces of dried, unprocessed marijuana or its equivalent every 30 days, and more is possible only when your practitioner certifies compelling reasons and the amount. You may possess up to a 60-day supply, which DHSS puts at 12 ounces. Adult-use buyers may purchase 3 ounces per transaction.
Is a medical card worth it now that recreational marijuana is legal in Missouri?
For many patients, yes. The state marijuana tax is 4% on medical purchases and 6% on adult-use purchases, and adult-use sales can also carry local taxes. Regular sales tax applies to both. Patients aged 18 to 20 also need a medical card to buy at all, because adult-use sales start at 21.
