Here is how the process works.

The short answer: get a written certification from an Iowa-licensed practitioner who treats your condition, apply online to Iowa HHS within 60 days of the signature with a photo ID and the $100 fee ($25 reduced), and watch for an HHS email within seven days, which carries a digital card if you are approved. The program runs under Iowa Code chapter 124E and is managed by Iowa HHS through its Bureau of Cannabis Regulation.

Iowa HHS notes that medical cannabis is not considered a first-line therapy for most medical conditions. Iowa’s rules are its own, and a card from another state does not let you buy at an Iowa dispensary.

Once your practitioner signs the certification form, you have 60 days to apply online with Iowa HHS for a state-issued card that lets you buy, possess and use medical cannabidiol legally. HHS replies by email within seven days.

To receive a medical cannabidiol card in Iowa, you first need a written certification from an Iowa-licensed MD, DO, PA, nurse practitioner or podiatrist who has examined and treated you. Adults 18 and older apply for their own card. House File 990 removed permanent Iowa residency from the statute’s card requirements on July 1, 2026, but the HHS rules still define a patient as an Iowa resident, so check with HHS before you apply.

Ask your practitioner to complete HHS form 470-0189; HHS must receive it within 60 days of the signature.

You need an Iowa registration card to buy from an Iowa dispensary; another state’s card does not count. Visitors with a valid card from another state may possess and use allowed forms in Iowa but may not buy here. To qualify for a medical cannabidiol card you need to be 18 years or older. Patients under 18 years old are required to have a registered caregiver.

I have a qualifying condition. Does it make me automatically qualified for Iowa’s Medical Cannabidiol Program?

No, simply having an eligible condition does not automatically qualify you. An Iowa practitioner who treats your condition must certify it. You’ll need to use the Iowa HHS online patient application after your healthcare practitioner has signed your Healthcare Practitioner Certification Form.

Where do I find an Iowa Medical Marijuana Doctor?

Start with the practitioner who already treats your condition. Iowa accepts an Iowa-licensed MD, DO, PA or nurse practitioner who is your primary care provider for it, or an Iowa-licensed podiatrist. Iowa HHS takes questions about the steps at 1-877-214-9313.

Can I grow my own medical marijuana if I have obtained my ID card?

No. Growing your own marijuana is illegal in Iowa, even if you have a medical marijuana card. Iowa allows patients only the medical cannabidiol forms its rules list, and no card covers home growing.

Does insurance cover medical cannabidiol?

Not by law. Iowa Code 124E.22 does not require Medicaid, private insurers or workers’ compensation carriers to pay for medical cannabidiol, so ask your insurer first.

How to Obtain A Medical Marijuana Card in Iowa?

Anyone with a qualifying condition and a written certification from an Iowa-licensed practitioner can apply for a medical CBD card. Patients 18 and older apply for themselves; for minors, a registered caregiver applies. See the practitioner who treats your condition, such as your primary care provider, and bring relevant medical records. If they certify you, they complete the state form. Finally, go to the Online Patient Registration Form within 60 days of the signature, and follow the steps on the Iowa HHS patient page to complete an online application.

How long is an Iowa medical cannabidiol card valid?

Your medical cannabidiol registration card expires one year after it’s issued. To renew your card, you’ll need to:

Visit your healthcare practitioner and have your condition recertified
Submit the same forms to Iowa HHS online, as in the registration process
Pay the renewal fee

When should I renew the certification?

You will receive a written or electronic notice 60 days before your card expires.

Is it optional to have a cannabis ID card?

To participate in Iowa’s Medical Cannabidiol Program, you’ll need to be registered with the Iowa Department of Health and Human Services and to be able to purchase from Iowa’s dispensaries you need a state-issued medical cannabidiol card.

What is the fee for this identification card?

The state fee for an Iowa medical marijuana card is $100 unless you’re eligible for a reduced fee. A reduced fee of $25 is possible if any of the following is provided:

Copy of an SSDI benefit notice or an SSI payment receipt
Copy of Iowa Medicaid member card, or your state’s equivalent
Government document showing veteran status

The state fee, full or reduced, does not cover the practitioner’s consultation fee, which is paid separately.

Is it okay to use medical cannabis while waiting for my Iowa Marijuana Card?

No. Wait for approval: Iowa HHS replies by email within seven days, and the approval email includes your digital registration card.

I lost my Iowa Medical Marijuana Card. What should I do?

If you have lost your medical card or it has been stolen, you’ll need to contact Iowa HHS to request a replacement. You will be required to present the photo ID you used when you applied.

What’s the process of renewal?

The validity of a medical cannabidiol registration card is one year from the date of issuance.

A cardholder who wishes to renew their medical cannabidiol registration card must submit an application and fee to the department. A cardholder who fails to renew their medical cannabidiol registration card may not legally purchase, possess and use medical CBD in Iowa.

What changes must I report to Iowa HHS?

Changes in your current residence address, name, or sex designation listed on your card must be reported to Iowa HHS within ten calendar days of the change. To change the current residence address, name, or sex designation listed on a medical cannabidiol registration card, you must give the card to the department and present a valid state-issued driver’s license, nonoperator’s identification card, or alternative form of valid photo identification that has been updated according to the procedures established by the state.

If I obtained my Iowa Medical Marijuana Card, who would know about it?

For patients and caregivers alike, participation in the program and any personally identifiable information are confidential under Iowa law, with the exception of a few limited circumstances. One example is a request from an authorized employee of a medical cannabidiol dispensary or a local law enforcement agency.

Only registered cardholders can legally purchase, possess, and use medical cannabidiol products from Iowa dispensaries.

What are the state fees for the Iowa Medical CBD Card?

Each application fee is $100 unless the patient qualifies for a reduced fee which is $25. Neither amount includes the practitioner’s consultation fee.

What are the possible reasons for a denied ID card application?

Your application can be denied for any of the following reasons:

The information contained in your application is illegible, incomplete, falsified, misleading, deceptive, or untrue.
The department is unable to verify your identity from the photo identification or other documentation presented.
You violate or fail to satisfy any of the provisions of Iowa Code chapter 124E.

What should I do if my application is denied?

You may appeal a denial or cancellation; 641 IAC 154.7 says appeals follow the HHS rules in 441 IAC chapter 2506. The written notice gives the reasons and the effective date, and a timely appeal suspends a cancellation until the contested case is decided.

Can a minor apply for the Iowa Medical CBD Card Program?

Yes. Qualified patients under 18 years old must have a registered caregiver.

How long does the card application take? Is it possible to expedite the application?

Iowa takes applications online, and HHS reviews each one and replies by email within seven days. A complete application, with the form signed in the last 60 days, avoids delays.

How would I know that my application has been approved?

Iowa HHS emails you within seven days of applying; an approval email has your digital registration card attached.

How can I participate in the program?

To qualify, patients 18 or older need a valid photo ID, the state fee and a certification from the Iowa-licensed practitioner who treats their condition; minors take part through a caregiver.

Is medical cannabis program participation publicly known?

No. Iowa law keeps program participation confidential, with the limited exceptions it allows.

Will there be more qualifying conditions to be added to the program?

It is possible. Any person may file a petition to add or remove a medical condition with Iowa’s Medical Cannabidiol Board. Within six months of the petition’s filing, or any longer period agreed to by the petitioner, the board must either deny the petition and notify the petitioner in writing of the board’s action and the reasons for it, or grant the petition and notify the petitioner that the board has recommended addition or removal of the medical condition, medical treatment, or debilitating disease to the board of medicine.

Caregivers

Who can be a legal representative or caregiver?

To be a medical cannabidiol patient’s caregiver you must:

Be at least 18 years old
Live in Iowa or a bordering state
Be listed as the designated primary caregiver in your patient’s healthcare practitioner certification form
Submit an online caregiver application to the Iowa HHS Bureau of Cannabis Regulation
Your application must include the following
Completed Primary Caregiver Application
Healthcare Practitioner Certification Form signed by your patient’s doctor, with you designated as the primary caregiver
A copy of your photo ID which can be either:
A valid driver’s license from Iowa or a state bordering Iowa (NE, MO, IL, WI, MN, SD)
A valid non-operator identification card
Payment of $25

Can a legal representative purchase medical cannabis?

Yes. A primary caregiver designated by the patient’s health care practitioner, who may be a parent or legal guardian, can buy medical cannabidiol products for the patient once Iowa HHS issues the caregiver a registration card.

Laws

Is Medical Marijuana Legal in Iowa?

Yes. In a limited form: registered patients may use medical cannabidiol from Iowa’s licensed program, with purchases capped at 4.5 grams of total THC per 90 days unless a waiver applies.

How is medical cannabis legal in the state?

For patients, only medical cannabidiol in the forms Iowa HHS allows is legal, and smokable flower is not one of them. According to 2020 legislation (HF 2589), there is a limit of a combined total of 4.5 grams of THC to a patient in 90 days.

I have a medical marijuana card. What if I encounter law enforcement?

As a patient or a primary caregiver, you should carry your medical cannabidiol card on you at all times when in possession of medical cannabidiol products. Products purchased at Iowa’s licensed dispensaries will have a label affixed to them with your patient number, which is also on your registration card.

Keep in mind that marijuana remains a federally controlled substance, and an Iowa card is not federal permission.

Should my employer know that I am a participant to the Iowa Medical Marijuana Card Program?

You must make sure that your employer’s policies allow the use of cannabidiol at work and/or outside of work. Take note that using CBD products even with low-THC content can result in a positive THC presence test.

Can I keep my CBD products if I'm in assisted living or long-term care?

Iowa Code 124E.23 does not require anyone who owns, occupies or controls a property, such as a care facility, to allow medical cannabidiol there. Ask the facility about its policy before you bring products in.

Dispensaries

Are there dispensaries in Iowa?

Yes. Medical Cannabidiol is currently available at five dispensaries across the state, in Windsor Heights, Sioux City, Waterloo, Iowa City and Council Bluffs. You can find them listed here.

Where do I purchase medical marijuana once I have my MMJ card?

You can purchase legal CBD products only in licensed Iowa dispensaries.

Can anyone purchase medical cannabis in the state?

No. Only holders of a valid Iowa medical cannabidiol registration card, or their registered caregivers, may buy from Iowa dispensaries.

How much medical marijuana can I purchase at once?

There is a limit of a combined total of 4.5 grams of THC to a patient in 90 days.
There are two exceptions to the 4.5 g per 90-day purchase limit.

For any patient certified for a terminal illness, a healthcare practitioner may recommend more THC per 90 days at the time of certification.
If the patient’s original healthcare practitioner certifies them for additional THC after the patient has participated in the program.

Your doctor has to fill out the 4.5g THC Waiver Form for this purpose. This form cannot be completed by a practitioner other than those who originally certified you.

Reciprocity

Does Iowa accept out-of-state cards?

A valid medical cannabidiol registration card issued by another state with a similar cannabidiol program allows an out-of-state patient to possess or use medical cannabidiol in Iowa, as long as the products meet the strict requirements of Iowa’s medical cannabidiol program. Patients from other states are unable to purchase products at Iowa dispensaries.

Can I use my Iowa medical marijuana card in another state?

Each state has its own reciprocity policy regarding cannabis patients. Make sure to contact the agency regulating the medical cannabis program in the state that you are visiting before traveling.

Iowa cardholders may also register as visiting patients in Minnesota.
