Dr. Green Relief sees patients in person at four offices: Tampa, Fort Myers and Sarasota in Florida, and Las Vegas in Nevada. Residents of the telehealth states listed further down can have the evaluation by video.

Which office serves you? Florida patients are seen in person at the offices listed below, because Florida requires an in-person exam before a first certification. Each Florida city card names the office that serves it. Las Vegas patients can visit our Smoke Ranch Road office.

Florida Offices: Tampa, Fort Myers and Sarasota
All three Florida offices are open Monday to Friday and closed Saturday and Sunday. Tampa: 4600 N Habana Ave Ste 32, (813) 575-2755, 9 AM to 5 PM. Fort Myers: 2540 Winkler Ave Ste 1, (239) 349-2999, 9:30 AM to 5:30 PM. Sarasota: 7250 S Beneva Rd, (941) 444-0064, 9 AM to 5 PM. At each office a qualified physician examines you in person before a first certification, and the state ID card is a separate application to the Florida Office of Medical Marijuana Use. Las Vegas Office
Our Las Vegas office, 7200 W Smoke Ranch Rd Ste 120, (702) 707-2414, is open Monday to Friday from 9 AM to 5 PM and closed Saturday and Sunday. A Nevada-licensed physician sees Nevada residents there in person, and Nevada law also allows an evaluation by telehealth.
