How Do You Get a Utah Medical Marijuana Card?

Utah Medical Marijuana Card Rules at a Glance
Here is how the process works.
To get a medical marijuana card in Utah, you apply in the state’s Electronic Verification System (EVS) with a UtahID, see a Utah recommending provider (in person for a first recommendation, with narrow exceptions), and pay the state card fee once you are certified.
Utah’s rules differ from those of many states: a first recommendation must be made in person with a few narrow exceptions, smoking cannabis is not allowed, and medical cannabis is sold only at licensed pharmacies.
Utah’s program is run by the Center for Medical Cannabis, whose duties Utah law moves from the Department of Health and Human Services to the Department of Agriculture and Food by January 1, 2027.
To obtain a Utah medical cannabis card, a Utah-licensed recommending provider must evaluate you and find a qualifying condition. To be eligible, you must be a Utah resident, at least 21 (or 18 to 20 with a petition the Compassionate Use Board recommends and the state approves), and have a qualifying condition or an approved petition for another condition. Patients under 18 need the same Board recommendation and state approval for a petition filed by a parent or legal guardian, who must also qualify for a guardian card or designate a caregiver.
AFTER A UTAH PROVIDER CERTIFIES YOU
Apply in the EVS, then get your initial certification from a Utah recommending provider, usually at an in-person visit.
Once a Utah provider certifies that you have a qualifying medical condition, you pay the state fee and download your card. Before your first purchase you meet the pharmacy’s medical provider, often a pharmacist, and smoking medical cannabis is not allowed in Utah.

Each provider may recommend cannabis to no more than 1.5% of Utah’s cardholders, a limit of 1,759 recommendations as of October 2026, and insurance-billed patients do not count toward it.
